"form fields only" won't print on a scanned PDF

I am attorney and when I file a complaint I need to use a blank form from the court that has a pressed seal on it.  I want to avoid having to use a typewriter every time, so what I'd like to do is: (1) scan the blank complaint into Adobe Acrobat, (2) fill in the person's name, address, etc. into the designated spaces using the 'Add Text' function, (3) place the sealed, blank complaint into my printer tray, and (4) print only the fields that I have entered (i.e., address, name, etc.) so that they overlay perfectly on the designated areas of the sealed complaint.  In theory, this plan works beautifully.

 

I've tried to add text using >Edit PDF>Add Text function, I add all the fields that I need to, and when I go to print "form fields only" the entire document is blank.  The only way those fields will print is if I choose "Document", "Document and Markup", or "Document and Stamp" from the Comment & Forms section of the Print screen, however this prints the entire form and I only want the person's name, address, etc. that I have filled out in Adobe (because I already have the blank form in my printer).

 

I'm using the Adobe Acrobat DC Trial and running Mac OS 10.9. 


Any help on this would be much appreciated!
